The process to schedule an appointment with a coordinated care specialist varies depending on your student status.
Returning Students
If you’ve previously met with the Coordinated Care Team, you’re encouraged to reconnect with the same Coordinated Care Specialist. You may also call the Engineering Advising Center (EAC) at 814-863-1033 to schedule a follow-up appointment.
New Students
If this is your first time connecting with the Coordinated Care Team, please complete the Coordinated Care Intake Form. After submitting the form, a team member will contact you to help develop a personalized plan for moving forward.
